Mass Intention & Card Request

Remember a loved one, living or deceased, with a Mass intention. Complete the form below and submit your donation by mail, place it in the collection basket noting that it is for a Mass Intention, or drop it off at the Pastorate Office.

Specify on the form below if a card is needed, and if so, whether you will be picking it up in the pastorate office or if you would like the card to be mailed (please include your full name and address in the comment section). If you are requesting a specific date, indicate in the comment section the date. The suggested donation is $10. Please keep in mind that Masses fill up quickly and the first opening could be 3 months out.  Please contact us as soon as possible if you are requesting a specific date to honor an anniversary of a loved one.
